Insurance Information

Meadowbrook Pediatrics P.C. participates in the following types of insurance:

Please call our office to discuss your individual insurance options. Our staff is very knowledgeable about medical insurance, however policies differ. It is your responsibility to know your coverage. Your policy is a contract between you and your insurance company, and you are ultimately responsible for payment of any bill. If you have indemnity insurance, we will provide you with a receipt, which has all the necessary medical information for your insurance company to process your claim. A receipt may be obtained at the time of the visit. If your insurance coverage changes please notify us as soon as possible. We will ask you to verify your current insurance coverage. If your insurance coverage changes, please have your new insurance cards available at the time of the visit. Remember to add your newborn infant to your policy immediately.

Some insurance companies require 365 days from the last physical exam (after 2 years of age) in order for the visit to be paid. Call your insurance company regarding their policy on this matter.

  • Managed Care (HMO) Patients

    • ​Please make sure that we are participating members of your insurance company. Remember your insurance company requires you to pay your co-pay at the time of the visit. Referrals are required in advance for all visits to other physicians or institutions. These referrals must be requested at least 48 hours before your visit to allow us time to fill out the forms. You may also request referrals via the Patient Portal. Please be ready to give the full name of the physician, diagnosis, procedure(s) to be done and the place of service. Most managed care companies refuse to allow us to give retroactive referrals. Please familiarize yourself with your company’s rules regarding Emergency Room visits.

  • Payment

    • Payment / co-payment for office visit is expected at the time of registration. A service charge of $15.00 will automatically be added if payment is not made at the time of the visit. The adult who brings the child in for our care is the individual we consider to be responsible for any co-pay. If you have any billing questions, please contact our billing office at 215-947-1447.

    • Returned Check – If a check is returned from your bank for non-sufficient funds there will be a $30.00 charge.
